Brooke Army Medical Center welcomes you to a new method for health care management—the Army Patient Centered Medical Home. Each of the patients enrolled in one of our seven Medical Homes (CPT Jennifer M. Moreno Primary Care Clinic, Westover Hills Medical Home, SPC Taylor J. Burk, Schertz Medical Home, Adolescent and Young Adult Medicine, General Pediatric Clinics and Internal Medicine Clinics) are assigned a Primary Care Manager (PCM) who helps them define goals and achieve improved health. This website contains information about the services offered by each Medical Home.
What is a PCM?
A Primary Care Manager (PCM) is a Physician, Nurse Practitioner or Physician Assistant who is your medical provider and the leader of the team that makes up your Medical Home. Your PCM helps you identify realistic health goals, and works with you to help you achieve those goals. Your PCM is also responsible for organizing, coordinating, communicating and monitoring your care across multiple setting such as with a specialty provider or support clinic.
Continuity of Care
Seeing your PCM at every visit is important to establish the provider-patient relationship that is the cornerstone of patient centered care. We will make every effort to give you appointments with your PCM. If for some reason your PCM is not available, you will see another provider from your Medical Home team who will have access to your medical record and be committed to honoring the plan of care you and your PCM have agreed upon.
