How to be seen in Pulmonary Clinic:

  • How do I get referred?
    • Have your provider submit a consult to the Pulmonary Clinic
    • Referral must be submitted through CHCS and/or
    • FAX referral to:
    TRICARE FOR LIFE patients: 1-877-875-0095 EXTERNAL REFERRALS (civilian PCMs): 1-877-548-1547 MTF to MTF (AD members): 1-877-861-5369 No Referral? Call 210-916-2153 and one of our staff will be able to help

Please allow 3-5 business days for the referral to get reviewed and processed. Central Appointment Management Office (CAMO) will contact you to schedule your Pulmonary consult. The CAMO office can be contacted at 210-916-9900 and select option for SPECIALTY CARE.

Please print and fill out the following forms (located on the left) and bring to your appointment:

  • Medication Reconciliation form (list ANY medication(s)/ vitamin(s)/supplement(s) you are taking that were NOT prescribed by BAMC.
  • ASTHMA questionnaire (if you have ever been diagnosed with ASTHMA)
  • Check-in form (fill out highlighted areas ONLY)

How do I schedule follow-up visits?

  • Please call the Pulmonary Clinic at 210-916-2153 to schedule your follow-up appointment and/or breathing test(s).
  • Please arrive at least 15 minutes prior to your scheduled appointment.
  • Please print and fill out the following forms (located on the left) and bring to your appointment:
