Medical Readiness Clinic (MRC)

• Complete service members portion of PHA:
  • Log onto (Google Chrome) https://medpros.mods.army.mil/portal/
  • Go to My Medical Readiness Portal.
  • Click on Periodic Health Assessment and complete member portion.
  • If you wear glasses/contacts, be sure to complete yearly eye exam with optometry. Call 210-916-9900 to schedule your eye exam appointment.
  • After completing member portion of PHA call 210-808-2588 / 210-295-4951 to schedule an appointment with a provider to complete part II of PHA process.
  • Part II PHA appointments are currently virtual appointments. Additional requirements for the PHA may be advised during virtual appointment. If the PHA is conducted face-to-face, the complete visit can take up to 1 hour. 
  • Please plan accordingly with Medical Readiness for the following, if needed for completion of the PHA:
    • Optometry:  Current and updated. If you wear glasses/contacts, take them to yearly eye exam.
    • Audiology:  Current and updated.
    • Age 35 and older, need fasting labs (Lipids and Glucose)
    • Age 40 and older, need EKG.
• Flight Physicals:
• Part I:  Call 210-808-2588 to schedule an appointment.  (6 a.m. - 3 p.m.)
  • Fasting lab work; Optometry (bring glasses/contacts); Audiology work up.
• Part II:  Clinic will schedule following completion of Part I packet.
• Military Physicals:  (Commission/WO, Airborne/RASP, Ranger, Special Forces, DOD/ROTC, SLC)
  • Part I:  Walk-in to receive prerequisite instructions and paperwork.
  • Fasting lab work; Optometry (bring glasses/contacts); Audiology work up.
  • Part II:  Clinic will schedule following completion of Part I packet.
• Retirement/ETS Physicals:
  • Part I:  Complete online on Medical Readiness Portal
    • Click on Medical Readiness Portal/Self Service
    • Click on MHA/PHA/DHA
    • Find the tab labeled SHPE and click ‘Start New Survey’ tab
    • Complete survey for DOD Form 2807-1, DD Form 2808, DD Form 2697
• Pre & Post Deployment Exams
  • Must get done 30 days or more prior to Real time; need labs and ANAM testing.
  • Complete associated questionnaires through online Medical Readiness Portal prior to scheduling an appointment.
  • Limited number of walk-in appointments with provider.
  • Post-deployment exams take precedent.
  • Part II:  Schedule appointment with provider by calling 210-808-2588 / 210-295-4951.
• Air Force can be seen in our clinic for in/out processing.  All Air Force physicals must be conducted at the Family Health clinic at Wilford Hall, or may be seen at the Randolph AFB clinic 210-652-2456. For Separation and Retirement Physicals call 210-292-7006.
• Guard and Reservists must be on orders for longer than 30 days or have Commander’s memo authorizing medical care.
